This form may contain personal information as defined under the Municipal Freedom of Information and Protection of Privacy Act. This information is collected under the legal authority of the Municipal Act, 2001, S.O. 2001 c25, as amended. This information will be used and maintained by the Municipality of Lambton Shores for administering the Municipal by-law enforcement and licensing process. Questions regarding this collection may be directed to the Municipal Clerk.

By submitting this form, I, the undersigned, hereby acknowledge and certify that:

  1. The short-term rental will comply with the Parking Management Plan
  2. The short-term rental is not subject to an order made pursuant to the Building Code Act, 1992
  3. The short-term rental contains functioning smoke alarms and carbon monoxide alarms as required in accordance with the O. Reg. 213/07 Fire Code
  4. The short-term rental will maintain fire extinguisher(s) and emergency exits in accordance with the Plan for Fire Safety
  5. The Owner, and Tenant if applicable, confirms they will require each renter to abide by the Renter's Code of Conduct
  6. The Owner, and Tenant if applicable, confirms the accuracy, truthfulness, and completeness of the information submitted; and
  7. The Owner, and Tenant if applicable, confirms that they will maintain liability insurance as required through this by-law while the short-term rental is being operated as a short-term rental
  8. The Owner, and Tenant if applicable, confirms that they will operate in accordance with all other requirements set out in By-Law 43-2022
